Breast Augmentation

Breast Augmentation uses silicone implants to increase the size or improve the shape of the breasts.
Indications
Many women consider having breast augmentation surgery in whom the breasts have not fully developed.
Various factors like age, genetics, pregnancies and changes in weight can lead to changes in the shape, size or appearance of breasts.
About the procedure
Breast augmentation surgery has been performed on more than three million women worldwide over the last three decades. Through the use of implants, this surgery can increase the size of the breasts and correct the uneven breasts. It may also be used to correct drooping breasts but may require an additional breast lift procedure.
Breast implant
The implants used in Breast augmentation surgery are filled with Silicone gel. Silicone gel-filled implants provide a more natural feel to the breast and are less likely to cause rippling. At present, most surgeons use cohesive gel silicone implants, which do not leak if the shell is damaged. Successful Breast augmentation surgery begins with careful planning to meet your particular goals. The second decision involves the choice between round and anatomical (teardrop-shaped) implants. The goal of this decision is to give you the most natural looking breast possible. Round implants have a tendency to show an upper edge, creating a “stuck-on” appearance. However, if there is sufficient amount of breast tissue and seeking moderate augmentation, this will not be a problem. Patients who are very slim, and have very little breast tissue to cover the implant may be advised to choose theanatomical implant to create a more natural contour. For patients who seek very large augmentation, disproportionate to their bodies, neither type of implant will look natural or attractive. In addition, there will be undulation in the skin of the lateral part of the breast, the condition is called Traction Rippling. You have to discuss about the size of the implant. This can be a confusing decision as a particular cup size will look quite different on two women of different stature. Finally, you will discuss the risks related to breast augmentation and what you should expect during your recovery.
Operation
There are three main approaches used for breast augmentation.
- The implant may be inserted through the crease under the breast (Sub-Mammary crease) using an incision approximately 8cm long. This is the most popular technique today.
- Access may also be gained through an incision on the inside of the Areola.
- A third method uses an incision in the armpit. Your surgeon will discuss these techniques with you and describe the benefits and disadvantages. Surgery will usually be done under general anaesthesia
You will be advised to wear a supporting bra, day and night for the first six weeks except while showering or bathing.
The following factors are important if planning for implant.
The size and Profile of the Implant, incision site (the area where the scar would ultimately remain), and the plane of implant placement.
After surgery
- The breasts will be tender and swollen for few days
- Postoperatively limit the arm movement.
- Postoperative haematoma may occur in the first 24 hours. This can be avoided by limiting the arm movements.
- Many people are able to drive after two to four days, depending on their level of discomfort.
Recovery
- Exercising too early can cause seroma, a collection of clear fluid around the implant. Avoid upper body exercise for first three weeks. After this you can start exercising gradually, with the goal of being back to your normal activity level after six weeks.
- There may also be changes in nipple sensation. These are usually temporary.
- The most common complication is capsular contraction, a condition in which the scar tissue around the implant shrinks and it feels firm or even quite hard.
- Infection can occur after breast augmentation. It will exhibit itself by swelling and redness appearing up to three weeks after surgery. If it occurs the implant will be removed and the infection is treated before the re-insertion of a new implant.
Regular breast self-examination is important for every woman after implant. If any lump felt that should be informed the plastic surgeon immediately.

Breast Augmentation with Fat Grafting
You need to have enough fat for breast augmentation with fat grafting. Breast augmentation will also “stretch your skin”. The fat can be reabsorbed into the body when you lose weight, which may reverse your breast augmentation.
Preoperative Care
The common medications that you are advised to stop three weeks before the surgery are Vitamin E supplements, anti-inflammatory medications such as ibuprofen, blood thinning agents and to stop smoking and drinking alcoholic beverages.
Post-operative care & Recovery
Recovery time from breast augmentation is 14 days. Patients should avoid strenuous activities during the first two weeks after their operations. After two weeks, they may do lighter exercise and slowly return to their normal lifestyles
